COMPREHENSIVE ASSESSMENT SPECIALIST

JOB TYPE:

Full Time Employment (a job working 40 hours per week or more)

 

ESSENTIAL DUTIES:

Conduct standardized InterRAI Assessments

        • Administer InterRAI assessments for initial waiver enrollment, redetermination (e.g. every three years), or in response to significant change of condition for individuals enrolled or seeking enrollment in developmental disabilities waiver services.
        • Collect information through interviews, observation, record review, and collateral contacts.
        • Coordinates and administers InterRAI assessments per Ohio Department of Developmental Disabilities guidance and WestCON procedures.

Complete Additional Assessments (as needed)

          • Conduct eligibility determinations using state-approved tools and criteria.
          • Perform waiting list assessments based on ORC 5123-90-04: Home and Community-based services waivers-waiting list and county board approved procedures.
          • Administer the Extraordinary Care Instrument (ECI) to evaluate intensity of care needs and support resource allocation decisions.
          • Maintain neutrality and consistency across all assessment types.

Interpret Findings and Support Person-Centered Planning

        • Collaborate with County Board, interdisciplinary teams, families, and providers to ensure assessment results inform individualized plans.

Engage Individuals and Families

        • Conduct assessments in a respectful, trauma-informed, and culturally responsive manner.
        • Clearly explain assessment purpose and encourage meaningful self-report and participation.

Maintain Accurate Documentation and Data Integrity

        • Enter assessment data into required software and agency systems within required timelines.
        • Ensure all documentation is complete, accurate, and compliant with regulatory and organizational standards.
        • Protect confidentiality in accordance with HIPAA and WestCON policy.

Maintain Certification and Professional Competence

        • Complete required interRAI assessor training and maintain certification.
        • Participate in ongoing training related to eligibility tools, waiting list processes, and the ECI.
        • Stay current with updates to assessment instruments, scoring rules, and state guidance.

Perform Field-Based or Remote Assessment Duties

        • Travel to homes, facilities, community settings or via telehealth as permitted.
        • Maintain professional boundaries and safety practices in all settings.

GENERAL:

    • Assure full and timely compliance with DODD rules, local, state and federal policies and procedures.
    • Other duties as requested or required that are considered legal and in line with the expected tasks and duties of this position.

    QUALIFICATIONS:

    • Bachelor’s degree in a related field required; a combination of education and relevant experience may be considered.
    • Minimum 18 months experience in the developmental disabilities field and 18 months experience conducting assessments, interviews, or evaluations.
    • Strong analytical, communication, documentation, and teamwork skills.
    • Ability to read and analyze written material and data for compliance with local and state standards.
    • Demonstrated initiative and ability to work independently with minimal supervision.Operate job-related equipment including the use of basic computer programs and systems.

     

    PHYSICAL QUALIFICATIONS:

    • Ability to sit or stand for periods exceeding one hour.
    • Ability to occasionally lift or carry up to 50 pounds.
    • Ability to perform simple grasping, light pushing/pulling, fine motor tasks, and reach above shoulder level.

    OTHER REQUIREMENTS:

    • Must possess or be able to obtain required Service and Support Administration certification from the Ohio Department of Developmental Disabilities.
    • Must maintain a valid Driver’s License and maintain a status of insurability for motor vehicle insurance. Reliable transportation is required.
    • Successful completion of a drug screen and BCI background check upon offer of employment.

    SALARY/BENEFITS:

    • Salary range for position: $48,508-$72,763 annually. Will commensurate with experience.
