Providers are required to accurately record the services they provide.  These records are used to submit billing claims and provider compliance reviews. Documentation is required each time a service is provided. Each service type has its own unique requirements for documentation. It is the provider’s responsibility to be familiar with documentation rules and stay up-to-date with any changes regarding service documentation.
