He lives with his parents and they are very involved in his life. He is his own guardian and is able to make his wants and needs known. He is a college graduate, smiles a lot, spirituality, has a blog on Holy Rosary web site, dedicated to assisting people with disabilities, and an active member of The Knights of Columbus. He must be in close proximity and able to be seen at all times. He enjoys attending his church Holy Rosary and it’s functions, he assists with RCIA classes, he is a board member of the little sluggers, and assists with their funds raisers and watching the Little Sluggers play baseball.

Nursing services – HPC services – He is dependent for all his personal hygiene needs. He needs to have digital stimulation for bowl movements. He is dependent for all day to day living tasks such as cooking, cleaning, and laundry. He is dependent for all ADL’s and IADL’s. He requires assistance maneuvering his electric wheelchair and needs repositioned every 2 hours. He is a choking risk. It is important for him to have assistance with all his ADL’s and IDAl’s. He receives assistance with transfers in and out of his wheelchair and assistance transferring and positioning. If an emergency and nobody was around he would be at risk because he would not be able to maneuver his wheelchair independently. Staff are not to use the Hoyer lift, as he is to unsteady on the lift. He needs certified staff to administer his medications. Staff or his parents also watch for when refills are needed. He takes his medication through his G-tube. He will need assistance with G-tube care and feeding if appetite is decreased, vest treatment, nebulizer, Percussion Vest treatment, respiratory assessment, blood pressure 2xs daily, medication administration through G-tube 4xs daily, bowl stimulation for movement. He is on a regular diet but must have his food cut up in very small pieces. He can drink liquids from a straw and staff needs to assist with exercise as he gets dressed daily.

40 hours Waiver Nursing 1st and 3rd shift weekly. 40 hours HPC services with some nursing services 1st and 3rd shift weekly.
